Our Office Hours are Monday -Friday from 8:00 -3:00p.m., excluding Holidays
Program Hours are Monday – Friday from 9:00 - 1:30p.m., excluding Holidays.
Bakersfield Community Health Center is licensed by the State of California. The Center offers medical and nursing care supervised by the participant’s physician; therapy; recreational programs; and social services to the frail elderly and adults over 18 years of age with physical , cognitive or mental impairments.
Eligibility for CBAS Program Includes:
- A medical condition that requires treatment or rehabilitative services prescribed by a physician
- Mental or physical impairments which handicap daily living activities but which are not of such a serious nature as to require 24-hour institutional care,
- Reasonable expectation that preventative service will maintain or improve the present level of functioning and high potential for further deterioration
- A Probable institutionalization if CBAS services were not available

Medical and Nursing Services
Ongoing monitoring and care by a registered nurse who works closely with your physician to ensure continuity of care.
Physical, Occupational and Speech Therapy Services
Designed to improve or maintain functional abilities in order to delay or prevent premature institutionalization.
Personal Care
Support and assistance is provided in the area of personal care and hygiene.
Recreational and Social Activities
Recreational and social activities are planned which are suited to the individual needs of the participants and are designed to stimulate social interaction and encourage both mental and physical exercise.
Social Services
Group and individual counseling are provided to help ensure optimum mental health. Case management is also provided with referrals to community services as needed.
Meals, Snacks and Dietary Counseling
A nutritious breakfast, snacks and lunch are served. Menus are created and overseen by a licensed dietitian.
Transportation
Transportation services are included for all ADHC/CBAS participants if needed.

If eligible, Medi-Cal may cover the costs of services. We offer a sliding scale fee rate for those without Medi-Cal coverage. Long -term care insurance may also pay for the costs for you to attend our program.
